How to register
If you wish to register with the practice please call in to see us.
You will be asked to complete a Registration Questionnaire and provide some identification:
- Photo ID (passport or drivers license)
- Proof of address (e.g. current utility bill, bank statement, pension statement - within the last 3 months)
If it will be difficult to produce these documents, please talk to us about other ways of establishing your identity.
Copies of the registration questionnaire are available from reception or click here to download it in advance of your visit. And you would also be very welcome to attend for a New Patient Medical with one of our nursing staff, an appointment for this can be arranged at the time of registration
If you normally live elsewhere in the UK, you may register temporarily with us for up to 3 months.
If you (or your guest) normally live outside the UK but are staying for longer than 2 weeks please enquire at reception with regard to registering with us.
We also offer various private medical services including consultations, medicals, letters etc. Please enquire at reception about fees.

Please tell us when you change your telephone number or address.
You can already book and cancel appointments on line and arrange for prescriptions via the website but you will soon be able to get results of tests online as well; please let us know if you wish to register for this service.
For the moment please ring us for the results of your tests. When you have your test your doctor or nurse will advise you of when your results are likely to be ready.
Please particularly remember to ensure we have the correct contact details when you have a test done, in case we need to call you urgently.
How to help with health costs
You can save money with a prescription prepayment certificate, also known as a (PPC)
If you know you have to pay for a lot of NHS prescriptions, it might be cheaper for you to purchase a prescription prepayment certificate.
A prescription prepayment certificate covers you for all of your own NHS prescriptions including any dental prescriptions and their is no limit on the amount of items that you need.
They can be purchase for a three month period or a year.
Three months costs £29.10 and a 12 month certificate is £104.00.
There are several payment options available and when paying for a 12 month certificate there is a monthly direct debit available.
You can apply online by clicking here and being directed to the PPC website, alternatively you can call them directly on 0300 330 1341. Or you can obtain a form from our reception.
Data Protection & Complaints Policy
Please remember to tell us if your name, address or telephone number changes.
Violent or abusive behaviour on the premises is seldom a problem, but cannot be tolerated, and would be dealt with under a code of conduct agreed with Crawley CCG.
Data Protection Act
Patient information is strictly confidential, and disclosed only to health care professionals involved in your care and necessary administrative staff who are bound by rules of confidentiality, unless you have given signed consent for details to be disclosed to a specific person or organisation.
You may inspect your records by prior agreement and on payment of a fee, as specified under the Data Protection Act.
Using our website you can book and cancel doctors’ appointments online. Please come to the surgery to obtain the necessary passwords. We need to see the patient who is registering for this service and photographic ID for all aged 16 or above. If you are registered for this service and need a new password we can't re issue this over the phone and would need you to attend the surgery in person.
Chasing appointments at the Hospital or services provided by another NHS provider.
If you have been referred by a Doctor or a member of our surgery team to the hospital or another NHS service but are worried you have not got an appointment, please call the service directly as they have a much clearer indication of waiting lists and appointments.
For all hospital appointments at Crawley, East Surrey or Horsham Hospitals please call 01737 784000 to speak with the Central Booking Office.
For all MSK and Physiotherapy appointments please contact them direct on 01293 600300 and ask to be put through to the relevant department.
For Mental health referrals enquires please call 01293 652300.

If you have recently had blood tests, swabs or urine test at the surgery please call us between 3 and 7 working days later. Occasionally tests are sent to distant specialist centres and may take up to a month; the doctor or nurse should warn you in that case to enquire later.
A receptionist will be able to tell you if the result has been marked by the doctor as normal, satisfactory, abnormal or "abnormal but expected"- they will also tell you what next step the GP has asked for if any. You may need to collect a leaflet/letter, or to make a telephone appointment or a face to face appointment or to repeat the test.
If a result needs URGENT follow up the GP or nurse will try to contact you directly. If it is not urgent they will leave the next step clear for you to hear when you call to get the result.
For all tests that are not done at the surgery but by another NHS provider please contact the service where you had the test done.
If a consultant has requested a specific blood test, CT, MRI scan, X-ray, the results will always go back to the requesting physician. In this case you need to call and speak to your consultant's secretary. If you do not have your consultant’s secretary’s details then you can reach them by calling the main switch board numbers for the appropriate hospital.

We currently have Dutch, Gujarati, Hindi, Punjabi, Tamil and Urdu speakers among our team. If you (or one of your relatives) have difficulty with English, if you are happy to do so please bring a friend / relative to help with translation. If you require us to organise a translator for you please speak to reception who will try to organise this. This will usually be on the telephone and will require a longer appointment than usual.

Procedure for requesting a chaperone
Langley Corner Surgery is committed to providing a safe, comfortable environment where patients and staff are confident that the best practice is being followed at all times.
All patients are entitled to have a chaperone present for any consultation, examination or procedure where they may feel one is required. If you require a chaperone to be present it would be a trained member of our staff.
Whenever possible we would ask you to make this request at the time of booking the appointment. If this is not possible we will try to provide a chaperone at the time of the request, however if a chaperone is not available it would be necessary to rearrange the appointment.
