Online Appointment and Administration Requests

For all appointment and administration requests, please complete a Patient Request Form.

Using these online forms will avoid the need for you to call the surgery and being kept on hold whilst we process many other calls.  

If you do not have access to the internet or have difficulties using computers or smartphones please continue to call the surgery and we will be able to support you.

You will be able to submit requests from 8am until we have reached safe clinical capacity or 3pm (whichever comes first) Monday to Friday.

We anticipate being able to respond to all requests on the day.

Phone lines are open between 08:30 -13:00 and 14:00-18:00

You will be asked for a brief description of the problem so that we can offer you the most appropriate response. It may be an appointment or we may suggest a local service that may be able to help you with your needs. For instance a local pharmacy’s or self-referrals. The reception team will fill out the form for you and it will be passed on to appropriate person.

Non-urgent advice: Cancel an Appointment

It is important that you inform the reception staff if you are unable to attend your appointment.

This will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters maybe removed from the list.

You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance. (excluding weekends and public holidays). You can also text to cancel your appointment.

Extended Hours Clinics

Langley Corner Surgery offers Extended Hours Clinics on specific days. 

These appointments are only available by prior booking.

GP appointments are now available during the evening and at the weekend for people registered with a GP in Crawley.

This means that both routine and urgent appointments are available at a local hub during weekday evenings and on a Saturday and Sunday, to make sure local people get the health help and support they need.

Sussex Health & Care and NHS Sussex, working in partnership with local patients and clinicians, designed the service. The appointment hubs are run by Alliance for Better Care Ltd, the local GP Federation.

How to book an evening or weekend appointment at a hub

Appointments can be booked through your own GP practice in the normal way, during normal opening hours.

Clinicians working at the hubs will be able to access your medical records with your agreement. You will be asked before any information is shared and can decline if you wish.

Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.

Please remember to update your contact details with us when you change address, telephone numbers and email address.

Home Visits

Home visits are only available for patients who are housebound because of illness or disability.

Please telephone reception as early as possible on 01293 514340. All home visit requests must be received before 12.00 noon.

A staff member may phone you back to clarify if your problem can be dealt with by telephone advice or to arrange a doctor or other service visit.

Chaperones

Langley Corner Surgery is committed to providing a safe, comfortable environment where patients and staff are confident that the best practice is being followed at all times.

All patients are entitled to have a chaperone present for any consultation, examination or procedure where they may feel one is required. If you require a chaperone to be present it would be a trained member of our staff.

Whenever possible we would ask you to make this request at the time of booking the appointment. If this is not possible we will try to provide a chaperone at the time of the request, however if a chaperone is not available it would be necessary to rearrange the appointment.

Referrals 

Chasing appointments at the Hospital or services provided by another NHS provider

If you have been referred by a Doctor or a member of our surgery team to the hospital or another NHS service but are worried you have not got an appointment, please call the service directly as they have a much clearer indication of waiting lists and appointments.

For all hospital appointments at Crawley, East Surrey or Horsham Hospitals please call 01737 784000 to speak with the Central Booking Office.

For all MSK and Physiotherapy appointments please contact them direct on 01293 600300 and ask to be put through to the relevant department.

For Mental health referrals enquires please call 01293 652300.
